Community Connection

What is Community Connection?

Community Connection acts as a bridge between healthcare systems and the community. When social determinants of health (SDOH) needs arise, FaithHealth Appalachia assigns each patient or community member a Community Connector. We currently have Connectors serving Cabell, Lincoln, Wayne, & Mason counties in West Virginia as well as Lawrence & Gallia counties in Ohio.
